Requiring the Department of Human Services, by May 1, 2028, to develop a certain form for the submission of a written report concerning suspected child abuse or neglect for cases in which the victim is currently at least 18 years old; requiring the Department to consult with certain individuals, entities, and the State's Attorney in the process of developing the form; requiring the form to include certain information including, if known, information on whether the accused individual currently has access to children; etc.
